THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.


Our Commitment to Your Privacy

Qi Oasis is required by law to protect the privacy and security of your health information, known as Protected Health Information (PHI). This Notice explains how we may use and disclose your PHI, your rights regarding that information, and our responsibilities under federal law.

We are required to:

  • Maintain the privacy and security of your PHI
  • Provide you with this Notice
  • Follow the terms of this Notice
  • Notify you if a breach occurs that may compromise your information

How We May Use and Disclose Your Information

We may use or disclose your PHI without your written authorization for the following purposes:

  • Treatment – To provide, coordinate, or manage your care, including communication with other healthcare providers involved in your treatment.
  • Payment – To bill and collect payment for services, including insurance verification and claims processing.
  • Healthcare Operations – For practice operations such as recordkeeping, quality improvement, licensing, compliance, and administrative activities.
  • Appointment Reminders & Health Information – To contact you regarding appointments, treatment alternatives, or other health-related information.
  • As Required by Law – When disclosure is required by federal, state, or local law.
  • Public Health & Safety – For public health reporting, preventing disease, reporting adverse reactions, or responding to health investigations.
  • Business Associates – We may share PHI with trusted vendors who perform services on our behalf (such as electronic health records or scheduling systems). These parties are required to protect your information.

Uses and Disclosures Requiring Your Authorization

We will obtain your written authorization before using or disclosing your PHI for:

  • Marketing (where required by law)
  • Sale of PHI

You may revoke your authorization in writing at any time, except to the extent we have already relied on it.


Important Notices

  • Fundraising: Qi Oasis does not use or disclose patient information for fundraising purposes.
  • Release of Records: Once your health information is disclosed to you or to a third party at your request, it may no longer be protected by HIPAA, and we cannot control its further use or disclosure.
  • Substance Use Disorder Records: Qi Oasis does not provide substance use disorder treatment as defined under federal law (42 CFR Part 2). If such services were ever provided, those records would receive additional protections.

Your Rights

You have the right to:

  • Access Your Records – Request to inspect or receive a copy of your health records. A reasonable, cost-based fee may apply.
  • Request Amendments – Ask for corrections if you believe information is incorrect or incomplete.
  • Accounting of Disclosures – Request a list of certain disclosures we have made.
  • Request Restrictions – Ask us to limit how we use or disclose your PHI (not all requests are required to be honored).
  • Confidential Communications – Request communication in a specific way or location.
  • Paper Copy – Receive a paper copy of this Notice, even if you receive it electronically.

Changes to This Notice

We may change this Notice at any time. Changes will apply to all PHI we maintain. The current Notice will be available upon request and on our website.
